A CHW serves as a liaison between the community and the healthcare, government, and social service systems. These are the people on the front lines, providing support to community members to help them improve their lifestyles and connect them with proper healthcare options. They assist patients with referrals, follow-ups, and completing applications for services.

Who Employs Community Health Workers (CHWs)
